A case presentation and overview of a middle-aged female with chronic piriformis syndrome who undergoes open surgical decompression and piriformis release. The posterior facet reduction osteotomy of the greater trochanter for trochanteric impingement is also described, as it is often done hand-in-hand with the piriformis surgical decompression.
